Position Summary:  This position is responsible for achieving and maintaining maximum census in the continuing care community including RCAC, Assisted Living, Memory Care and Skilled Nursing.  This position will regularly apply sales and marketing principles when interacting with healthcare professionals who include case managers, nursing homes, hospitals, home health agencies and physicians.

 

 

Essential Duties and Responsibilities:

  • Implements sales plan that actively markets continuing care services to internal (short-term rehabilitation patients) and external prospects.
  • Responsible for assisting referral sources in identifying patients with care needs that match the services of the Cedar Crest, Inc. Community.
  • Serves as a representative when interfacing with hospital discharge planners, acute care staff, physicians (both internal and external) and other professional referral sources.
  • Provides on-site pre-admission clinical assessment services to referring providers.
  • Educates the potential resident and family on services provided, the admission process, treatment process and projected outcome.
  • Communicates with the appropriate nursing staff and ensures the community is prepared for the resident’s arrival.
  • Coordinates the resident’s transfer to the community and completes any necessary transfer and admissions paperwork.
  • Effectively manages relationships to achieve a positive process with referral sources and referring providers. This position is the primary source for information regarding current services.
  • Conducts regular visits to referring hospitals to build and maintain relationships with the Discharge Planners, the Case Managers, Physicians and others.
  • Coordinates tours and admissions conferences for potential residents and their families.
  • Responds to customer inquiries (phone, on-line and walk-in), answering questions about the community, the priority list, reservations process and the admissions process.
  • Serves as the community representative to Cedar Crest residents that are temporarily receiving treatment in an outside facility.
  • Fulfills other duties as assigned by the Director of Nursing.

 

Required Knowledge, Skills, Abilities and Experience:

  • RN or LPN licensed in good standing in the state of Wisconsin or a Bachelor’s degree in a related field is preferred.
  • Prior healthcare related sales experience preferred.
  • Excellent interpersonal skills and the ability to relate professionally with referral sources, residents, family and co-workers.
  • Demonstrates excellent verbal and written communication skills.
  • Demonstrated work history of being a self-starter, working independently with minimal daily direction or supervision with results oriented goals and expectations.

Pay is based on experience up to $75,000.
